We take pride in knowing our patients and in meeting the personal need of each patient we serve. When making an appointment, please inform FHC staff of the reason for the visit, the nature of the problem, and the provider you wish to see.

However, children under the age of nineteen living in your household that are not covered by "dependent" insurance through a parent or guardian must apply for a Medical Card and CHIPS through the WV DHHR before applying for the Sliding Fee program. If denied for both services a denial letter from each service must be presented with your Sliding Fee application before it can be processed.

The women's health here is absolutely horrible. Doctor Cheek was mean and not helpful. It's a shame because the people who work with her were very nice. Do not waste your time. Go to Charleston or Parkersburg instead it's worth the drive. They need to have her retire and hire someone else from the 21st century who is sensative to women's needs and questions.
