The mid 1980s brought an economic revitalization to Salt Lake City's downtown that included the demolition and redevelopment of many substandard housing units otherwise known as Single-Room Occupancy Hotels (SROs). The SROs were homes to Salt Lake's lowest-income residents who often worked odd jobs within walking distance as janitors or watchmen. When the roughly 800 housing units were torn down, 1,000 residents were cut off from their homes and jobs. Prior homelessness in Utah was an isolated and temporary phenomenon.

Services are offered on a sliding fee scale. You may need to pay a nominal fee for services, based on household income and family size. If you have not been seen at Fourth Street Clinic in over three years, come to the Main Clinic Entrance on 400 South for new patient registration. You are a Registered Patient if you had a Fourth Street Clinic appointment within the last three years.
Our core values have driven our mission-to provide high-quality health care and support services to homeless Utahns-forward for more than 30 years. Over 60 dedicated staff members at Fourth Street Clinic value respect, accountability, trust, integrity and transparency when serving the underserved. In 2019, we expanded, adding new programs and services to increase our impact.
All volunteers must complete a background check and fingerprint screening-for a cost of $38.25, plus the cost of fingerprint screening. 4. As of summer 2020, Fourth Street Clinic isn't accepting any medical in-kind donations. Fourth Street Clinic received almost 50% of its total funding from in-kind donations and volunteers in 2017. An in kind donation plays a huge role at Fourth Street Clinic. At this time, we won't be accepting any baby or children items such as clothing, diapers and toys; you can contact The Road Home, The Road Home Midvale, or The Children's Center to donate any children's items.We also don't accept catheters or medication of any kind.

I love the dentist. Dr Schneider you are amazing, thank you for what you did for me, your not only very pretty, but you are amazingly talented and professional. You are a blessing to say the least. Tammy I can't and wont forget you either you had alot to do with getting me in there. So thank you to all the staff at the 4th Street dentist. I was told to do a walk in to see a doctor when I got there they sent me home without seeing anyone, and told me to call and make an appointment, when I called it said I was number 1 in the queue the it would say that they were busy helping other people and to leave a detailed voice-mail with my name and birthday and why I was calling so I did for 2 weeks straight.
I finally had to go to the emergency room where they admitted me to the hospital and amputated half my left foot. The 4th street clinic finally called me back after I was admitted into the hospital. More than 2 weeks went by before they got around to returning my calls.

Wonderful services and experience with Fourth Street Clinic. Shoutouts to: Sage, Josh (phlebotomist), Leah (window 4), and also the receptionist who did my registration at the front desk, and the nurse who walked me back to the room, but who's names I sadly forgot. Dr. Chris Belknap was my Physician and his demeanor really put me at ease talking with him.
He was personable, had a willingness to listen, curious, and empathetic about my health concerns. All of the staff of whom I mentioned were very kind, helpful, efficient, professional, and went above and beyond their duties. I had a pleasant experience with my visit and I HIGHLY recommend this clinic. *Especially for the homeless as they don't make you feel judged, profiled, or awkward, unlike other places I've been. And with that being said, the building, waiting room, and examination room were clean, peaceful, quiet, and not what I expected from this clinic.

This clinic does amazing work for folks who are either homeless, or transitioning out of homelessness. I am working there as a student-intern, and have seen first hand the compassion, caring, and top-notch services they provide. What's amazing is that these services are all completely free to the patients. Generous folks in the community have donated money so that Fourth Street can offer free medical visits, medications, medical supplies, mental health services, and wellness groups to some of the most vulnerable people in our area.
The directors at Fourth Street have managed this clinic so well that it looks and runs as smoothly as clinics that serve patients with good insurance in nice neighborhood. The medical providers have compassion and dedication to help all patients, regardless of poverty, addiction, or mental health challenges. If you know someone who needs medical services, and they struggle with housing, send them to Fourth Street.
