Jump Start offers a nutritionally balanced menu plan that combines a wide variety of protein-fortified prescription foods, along with a healthy meal of the patient's choice. JumpStart emphasizes high-protein and lower-fat foods. New Beginnings is a medically-supervised weight loss program individualized for each patient.
All of us at Monarch Medical are sensitive to the ever-spiraling cost of your healthcare. We congratulate you for recognizing, as we do, that weight management is a central and important medical issue. Many of our patients have had their program covered by a health insurance plan. As a courtesy, Monarch provides benefit checks to estimate what your carrier covers for our programs.
We prescribe vitamin injection kits for self-injection. Homekits are shipped to your home after a review of your medical history by our Practitioner. Every kit is a 30-day supply. A Telemedicine consultation with a Practitioner is required and included before the shipping of each order. Homekits are currently only available for Oregon residents.

After waiting almost 2 months for a first visit appointment, I asked several times "how much is this going to cost?" the assistant and the nurse told me nothing that your insurance will take care of it all. I was never asked to sign an agreement and the forms I did sign for approval to be on the program, they said I don't need a copy of. When I went in for my 1 month visit I was told that the B12 shot is included and the vitamins.
When I went to check out the front desk told me I would need to pay $400 or just make a payment. I asked again, "I thought my insurance was paying for this", they then said you have to meet your deductible which was never mentioned before. I made a payment and when I got home I called MaryAnn the finance person (which is not working there any longer) she told me that she will waive my monthly visit and discount some off the initial visit since I was not given any information about the cost and about my insurance. At this time, I will NOT be going back and I DON'T recommend this facility at ALL. It's a shame that a business can do this to someone.

When I first started the program (around the beginning of 2017) it was, at that point, my last among many attempts of losing weight. By September of that same year I had lost close to 85lbs, and I have been able to maintain the new weight, even close to 4 years later. At this point I only go in to get specific foods from the marketplace once every week or two but if it weren't for this program I don't think I would've ever lost any of that weight.

I was billed when I was told my appt would be covered by my insurance. I left a message in Jan when I got the bill, no return call, then I got another bill and talked to someone in Feb. was told the visit was billed incorrectly and she would take care of it. I continue to get billed and am about to be taken to collections! I am VERY angry! This is beyond ridiculous!
Will not trust this place again or recommend to anyone! I dont have 275.00 playing around to give away! Also. I have a voice worker who has the same insurance, had the same initial visit the same week, and did not receive a bill. We both asked about the cost before coming in, and both of us were told our insurance would cover 100 %!

On 2-19-20 I had my 1 month appt at 9AM with Monarch. To this date I was told that my insurance covered everything. I had no charges on the prior 4 times I went in. On this day I was told that I needed to pay $143.+ to get the vitamins, shot, and see the FP. I told them I didn't understand the insurance and also what was in this so called "shot" that they push each time you go in.
The assistant said speak with the FP. Them I said I would call my insurance co. Took about 5 minutes and then at 9:17 AM the assistant came in and told me to make up my mind as I was taking space in the chair and I responded that you are basically kicking me out and she stated not really but your appt. was at 9AM and make a decision if I want to continue with the program or not.
As if no one has ever had to wait from the weigh in room to the FP room for the FP. This really upset me and at that point I decided that I wasn't welcome to take any more of their precious time in the chair. I left and not going back. If I could I would give them a no star rating.what a way to treat a customer.

If I could give this review no stars I would. I was excited when I called to inquire about this opportunity that soon changed after my second appointment. When I called for my first appointment I was very concerned with cost. So I asked the lady on the phone how much my total appointment would cost, she responded with a very reasonable price to I reworded my question to clarify she repeated the same number and confirmed that that was all I was going to be charged for my appointments so I went and started my journey.
It wasn't until after my second appointment that made me so angry and defeated. I had seen the nurse practitioner Angela Zacharek at this second visit in which she said " you are doing good I don't think there is anything we need to talk about." and the appointment was over. As I was walking out of the room into the waiting room the receptionist speaks loudly stating " I need you to come and speak to the billing department right now about your past due balance." ( news to me as I had paid the discussed cost cash after my first visit and this is the first time I was notified that I owed them anymore money!) So it was a very embarrassing to say the least.
So there I was standing at the receptionist desk in front of the whole waiting room speaking to Jodi their billing person about a balance that I had no clue about. THIS BALANCE WAS 10X THE AMOUNT I HAD AGREED UPON. Throughout this conversation I was forced to stand next to fellow patients that where checking in or checking out. As the phone call ended I was confronted by the " office manager" SHARLIN MELTON MA who once again shared all my information for everyone to hear and proceeded to tell me that I now could not leave the office with the supplies that the Nurse Practitioner ANGELA ZACHAREK had given me.
I had already placed these supplies on the counter as I did not want any reason for them to charge me any more money. When I explained that to SHARLIN MELTON MA she stated " OK then HONEY you can leave now!" So now I'm left without answers or knowledge as to why. This whole operation is appearing to be fraudulent.

I had great success with the program, but it's time to go back and get back on track. I called and since I was there last, they started taking insurance. The monthly visit has gone from $99 plus food and meds to $179 plus food and meds. Very disappointed that they went up so much due to insurance. I wish they could charge differently for those who don't have insurance.
So off to my own doctor to get the same meds that don't cost as much and all I miss is talking to a nurse once a month. Sorry Monarch, had to go to another facility to get the same results and all for a $20 co-pay. I would recommend to anyone who can't afford the $179, that turns into about $300 after food, meds and shots per month.now I spend about $100 per month, maybe a little less.
So off to my own doctor to get the same meds that don't cost as much and all I miss is talking to a nurse once a month.
