ASSIGNMENT OF BENEFITS: The above information is true to the best of my knowledge. I authorize my insurance benefits to be paid directly to Polley Clinic of Dermatology. I understand that I am financially responsible for any balance. I also authorize Polley Clinic of Dermatology, its related companies to release medical information required to process claims and to release my medical record information to provide care. I authorize pictures of myself and of clinical focus areas to be stored in my medical record.
If we do not have a contract with your medical insurance company, or if you do not have medical insurance, payment in full is expected at the time of your visit. You will be required to show a copy of your insurance card at each visit. I really like this dermatologist and the clinic. Dr Polley is going to try different options for you if one doesn't work. He doesn't rush the visit, and his Team is very professional. I think anyone going here will enjoy the experience and get top quality care from this Team. I'm glad I was referred here.

Everyone was very friendly and helpful. I had a wonderful first dermatology appointment at 40 years old. GPS led us to the back of the building which I didn't know at the time, and there was no sign directing where to go which was a bit confusing. On my way back to my vehicle I also met a couple trying to find their way as well. This is only a recommendation and doesn't in anyway affect the service and care I received as it was beyond exceptional!

I had an appointment to be seen for acne after recently giving birth to my child. I paid my co payment upon arrival to the clinic. The staff is very courteous. I waited a hour and 45 minutes past my appointment time to be seen for about 2 minutes by the doctor. I was prescribed a face cream. And at the time the doctor said Tricare may not approve it.
Which he was correct. I never received my actual medication or offered an alternative to which would be approved. I received a bill in the mail about 3 weeks later for a full amount because they did not process my insurance. I called the billing office to let them know I had insurance. I received another bill about 2 weeks later. Still showing the same amount, I called again to let them know I had insurance.
I then received another bill showing the insurance pay their amount and I had payment left plus a late fee. I then call the office again to ask if they can waive the late fee. I was told no because I received multiple bills already. I tried to explain that this was the first bill I received where my insurance actually covered the amount they should have so I felt I should not have a late fee.
They denied my request so I went ahead on and paid the full amount that they said I owed. I have no problem paying my patient portion because I was seen but I do have a problem when I have to pay a late fee and never received actual medication for what I was seen for. I will not be returning to any Polley Clinic for future dermatology needs.

The Dr Denise is great. Receptionists at the FRONT counter are professional, sweet, kind and welcoming. The other male Dr comes off cold and not at all welcoming at all from what my friends and my own child i witnessed and told me they experienced. Theyre always late / running behind. I came almost 20mins b4 my scheduled time, becuz they ARE ALWAYS running behind to a point where i have been called THIRTY-One minutes aftr my appt time.
Interesting because, Dr's office can be late and fall behind, but if u are, Its a rescheduling. I say, its horse s**t! I would NOT recommend coming to this facility if you want accurate service. The 3rd party phone service is horrible process, as you have to wait for them to call the local office in your area, send a message and days gone by and still no reply from the branch office.
The communication on that rather than directly to the local office for direct contact/ communication is absolutely ridiculous. Unethical, especially when dealing with someones health and or medication. Very sluggish service. Not calm, straight sluggish. Time and service efficiency is NOT a factor. Procedures go smooth but the cons, aside the pro to this facility everywhere else is NOT at all worth it.
The waiting line is so soooo far from the check in and out window that you have to basically wave them down. The "wait here" marker should be moved at least 2 feet closer for someone to know your even there without rubber necking around the window to be noticed with not too much of a wait. I despise the wait. Then when they do call you back they try and rush your appt/ needs due to them running behind.
My slot time for a procedure cite check follow up shouldve been 15-20 minutes. i was called back and screened 19mins after my scheduled time, to then be told to stand by for the Dr. I tried to address some concerns and was informed, "sorry but u only have a certain time for appts." However, they are the ones stripping a patient of their covered / paid time slot allotment. Dr. didnt arrive to my room until 34 minutes after my appt time slot was scheduled.
He was done with me 5mins later. Granted Dr did ask me if i had any concerns and took the time to listen, but overall its the principal of the matter that the wait from your appt time is uncalled for.

From my own personal experience, this place is great! I haven't been to any other dermatology, but within a month of my lifelong struggle with my skin, I started to see real results! I was beyond happy and I felt truly pretty for the first time. That was in February 2020. Now in March 2022, the same prescriptions are working phenomenally. The only complaint I would have is that I am at a standstill.
Every time I go, it's only for the refills since there's nothing I can do to clear the scarring on my skin at this point. I keep being told that they will fade with time which I understand, but if there is an option to have a virtual appointment for this reason, I would love even more. My drive is 2 hours to get to the appointment. I would also suggest recommendations on what else I can do for my skin since my appointments are seemingly redundant only for refills. Seems pointless to get there and spend under 5 minutes to check up on my condition and get the same refills each time with no updates to conditions or anything. I digress, I still believe 5 stars is worth it here from my own experience! Keep it up!

I do not recommend this place, I have a problem some time ago in my scalp and in my private parts, the doctor did not take much 5 minutes to check me and formulated a shampoo and a topical cream, I told his assistant that this was not It would help me because I had already used it before and he suggested that I also use the topical cream on my scalp when they are two totally different and different areas to treat. not recognizing this place they only seek our money and not cure the patient.
