Hamilton Community Health Network offers comprehensive healthcare services including adult and pediatric primary care, dentistry, obstetrics, gynecology, mental health, podiatry, pharmacy and vision services. Language help is available on request, along with 24/7 provider access and after hours assistance, call 810-230-3840. This health center is a Health Center Program grantee under 42 U.S.C. 254b, and deemed a Public Health Service employee under 42 U.S.C. 233(g)-(n). Services
Hamilton Community Health Network has Certified Application Counselors (CACs) who can assist you with your enrollment needs from signing up, to filing an appeal or exemption. Our CACs assist with Medicaid plans such as the Healthy Michigan Plan and MI-Child or Healthy Kids, the Genesee Health Plan, and the health plans offered on the Marketplace through the Affordable Care Act.
Hamilton patients who are over the age of 18 and are living with another family or adult can often qualify for additional healthcare assistance. This includes college students who commute or seniors who live with their adult children. Many times, these 'homeless' patients can get free medical care and even low-cost dental care, vision, and pharmacy benefits.
We will try our best to get you an appointment when you need it. We offer walk-in hours and same day appointments at all of our clinics. We may verify some information over the phone, including how to apply for the sliding fee program, if you are eligible. Please arrive at least 30 minutes before your appointment to complete the paperwork and registration process.
