Mountain Family's roots date to 1978 in Black Hawk, Colorado, when the area's only physician retired. Citizens formed the Gilpin County Health Clinic, later becoming Columbine Family Health Centers, with clinics in Black Hawk and Nederland.
Columbine's success was noted by towns west of the Continental Divide who saw their own medical resources dwindling in the 1990s, when the area's only pediatric practice willing to accept Medicaid suddenly closed its doors.Asked to open a clinic in Glenwood Springs, Columbine agreed despite the challenges of distance and weather, honoring its mission of ensuring health care for Colorado's underserved.

Since 1978, as others closed their doors to those without private health insurance, we have continued to open ours. From a single clinic we have now expanded into a network of health care facilities along the I-70 corridor from Parachute to Avon, and along the Roaring Fork Valley to Basalt. As a nonprofit, community-led, Federally Qualified Health Center system, we are dedicated to providing comprehensive, integrated health care for all. We firmly believe health care is a basic human right, not a privilege.
Need help with your health insurance application and applying for financial assistance? Special enrollment periods are still available to those with life changing events (did you lose coverage, get married, have a baby, etc.?) If you need help navigating the enrollment process, give us a call at 833-279-6627 or email us at EnrollmentHelp@mountainfamily.org.
Mountain Family Health Centers is using a new vendor to handle our patient billing process. Soon you will receive statements from Instamed where you can use a QR code and go directly to signing up to pay your bill on Instamed. We will keep our current payment option available for a few weeks during this transition, please choose to use either payment option to complete your payment.
Mountain Family works hard to increase the availability of dental care in our region. We offer full-service dentistry at our Rifle clinic and our Mobile Dental Van in El Jebel, as well as preventive care at our SMILES Dental Project locations. To further increase access to care, in 2018 we will expand our Basalt clinic to include full-service dentistry.

I am always disappointed with the staff. I like my doctor but I'm considering moving to another clinic. The front desk constantly send the phones to the answering service at random times during business hours and if you do leave a message no one will get back to you. I've had several tele health appointment that I've missed because I needed a new link to the appointment but couldn't get ahold of the front desk.
I have spent days waiting for scripts and calling everyday but one will actually send it. The staff has no follow through, if you ask for a call back or leave a message they will not get back to you.

I've read reviews, and every time I have an appointment I am greeted with smiles and respect. And the ladies at check in desk call me by my name. When I wait in line and get to counter I make sure I give them name, date of birth, address, and phone number, and whom I'm there to see and time of appointment. And things flow easily. I'm checked in and asked to take a seat.
People come in with attitude, and think there better than the people waiting. Dr Aguirre is a wonderful Doctor and is always on point with me and my family. Be on time and make things easier or if your running late give acall. There at this place to help you. Stop with the attitude and relax. Casey is the Doctor to see. Wounderful person And if people attend to there children and respect and put things back there should be no problems. It's your choice or go elsewhere. Give a little get more back.

If I could rate this place less than one star I would. The lobby is disgusting and you can't even find a clean chair to sit on. The front desk ladies were also rude. We got rushed through our appointment and it was the first time that a doctor didn't show us an x-ray after getting one done. We are going to another doctor next week to get a second opinion because of how horrible our experience was. (Glenwood Springs location)

I have Made two appointments to see the Psychiatrist/ Psychologist and both were canceled the day of my appointment. Now they want to set up a third appointment another month away! Every time these appointments are canceled has been due to the doctor calling in sick! When you have socialized medicine and I am a doctor at that clinic i would never work either! Pathetic, disturbing service to the local community, this place should be ashamed of themselves!

I am not convinced this place truly cares about your health, i feel like they are just in this for the money. I taught my son to never let strangers touch you down there, doing a well check up doctor checked his genitals and made him uncomfortable, then had the nerve to make it sound like we his parents violate him and even got a counsler involved.
My son is the only one who touches his penis, of course when the doctor touched it he felt extremely violated. He's old enough to know that only he should touch his private parts, so of course he was uncomfortable. The only one who violated him, was the doctor. I take care of my kids and i do not let anyone harm my children. I feel like i failed my son by allowing the doctor to make him feel uncomfortable.

While traveling through Colorado, our daughter fell ill and needed medical treatment. With no urgent care facility around, we turned to Mountain Family. Despite us not being patients, Mountain Family fit us in within MINUTES of our call (by chance, we were staying right next door). Our daughter received excellent, personalized care from Dr. Claire Graff, who considered our individual needs as a result of our travel and tailored her treatment accordingly.

Mountain Family has helped me and the people I love when we have needed it most. Dental, medication, infections, understanding Medicaid and CICP, even therapy. I truly value them and I am thankful they are there for us. They are very popular and very busy. They may not always be on time but they're trying to help so many people. I'm glad to have access to a wonderful organization full of kind and helpful people.
