Upon submission, your answers can be reviewed by the physician and then imported directly into your records. The Patient Health History Form, Patient Registration Form, and Universal Consent are required for your first visit. You do not need to complete both the online and printable Patient Health History. Please choose whichever version you prefer.
Dr. Morrell spends the time necessary to get a thorough history and perform a comprehensive physical exam, knowing that an accurate diagnosis and timely treatment are the foundation of the best treatment plan. Doctor Morrell is a board-certified orthopedic surgeon and is fellowship-trained in traumatology.
Doctor Sean Odell is a Board Certified orthopedic surgeon who is fellowship trained and board certified in sports medicine and arthroscopic surgery. Dr. Odell specializes in the treatment of complex injuries of the shoulder and knee. He commonly performs arthroscopic surgery of the knee for meniscus tears and ACL reconstructions.
Doctor Friedman is a board certified podiatrist and earned additional certifications in endoscopic and shock wave treatment for plantar fasciitis. He specializes in all foot and ankle disorders resulting from injury, overuse, deformity and degenerative conditions. He also specializes in ballroom dance injury and injury prevention, customizing special orthotics for ballroom dance shoes.

I had some unexpected experience with DR. ERWIN FRIEDMAN. My daughter had a pair of shoe insert custom made this summer while she spend few weeks at home from College. We finally ship her the insert in Aug, she keep telling us it does not fit in any of her gym shoes. I keep telling her try harder. This Nov thanksgiving she brought it back home, we went shoes shopping at DSW for entire day looking for a pair of gym shoes that would fit the insert and no lucky there.
I then went back to Dr. Friedman, asking for remake another pair. The insert is too wide at the mid point where the arch support is needed. You can not trim the arch support off, that would totally defend the purpose of inserts. He asked for me to come in with few pair gym shoes, let him try. He spend about 30 mins to try, it does not fit. He then tell me he wants me to go to New Balance store to look for specific shoes that he believe it will fit.
I told him, we went ahead made this pair of shoes insert based on his practice staff promise that the insert will fit in any gym shoes, including the All Star flat she had on that day. We are expecting this pair of insert can be moved from one pair to another because young girl don't just own one pair of shoes. I am very disappointed at the treatment by Dr. Friedman.
He tells me he never did tell my daughter that inserts fits any gym shoes. I am warning you, ask clear questions before you commit with this practice. There is no regret service, there is no remake, there is no accommodation and they do not remember what they have told the patient before committing to their service. There goes $600 inserts, I left for Dr. Friedman as a gift.
