Weil Foot & Ankle Institute was founded in 1965, by Dr. Lowell Weil Sr, who was inspired by a need to progress the Foot & Ankle Care category into the future through innovation. As one of the first Doctors of Podiatric Medicine (DPM), Dr. Weil Sr. trained around the world, seeking out ideas, both within podiatry, but also outside of that realm, to dream of new ways to make podiatric care better.
Weil Foot & Ankle patient information includes detailed information on your visit, patient forms, new patient information, and insurance information. We require all patients and visitors to enter our facilities with a face covering that covers both nose and mouth already in place. If any person arrives without a face covering, they will be asked to return to their car to put one on or reschedule their appointment or visit. If you are a new patient, please arrive to your appointment early in order to complete the registration process.
Are you from out of town? Weil Foot & Ankle Institute offers same-day appointments to treat your urgent foot and ankle needs. You can avoid a visit to the emergency room by making an appointment at one of our convenient locations. In addition to 20 Chicagoland locations, we also have offices in Wisconsin, Michigan, and Indiana.
Prairie Path Foot and Ankle Clinic is now part of the Weil Foot & Ankle Institute family. Nice clean location, beautiful staff. I had surgery last year August of 2012. Dr. Baker is phenomenal, the surgery went well. After surgery no pain meds was taken even though I had them but no pain endured while my foot was healing. I recommend anyone that is having foot issues to go to him. He knows his work and is very good, highly recommended on my end.

Dr. Baker and Dr. Cabai and the team at Weil have done a fantastic job with my ankle treatment. I broke my ankle in two places with extensive tendon damage in the winter of 2021 - both Dr. Baker and Dr. Cabai listened to my health goal of wanting to continue to lead an active lifestyle and we chose a great approach to accomplishing that. We just completed my final post operation check in and the ankle and foot have healed as well as we could have expected and I am thrilled. Outstanding job by the team here - I would recommend this practice to anyone seeking foot and ankle treatment. Thank you so much!

This was my first visit to Dr. Siddiqui's office, so I wasn't certain of what to expect. But, right from the first, the staff were courteous and helpful. Then after a very brief wait, I met Dr. Siddiqui whose demeanor and professionalism were very reassuring. He gave me hope that my medical needs were finally being addressed in a highly competent manner. Needless to say, I look forward to my next appointment with him.

I am getting physical therapy from Monica at the Lincoln Park office. After just 2 visits, the persistent discomfort in my ankle and hip has already subsided. She understands the causes of my problems and addresses them with both targeted massage and exercise. I like the two-table PT area that is clean, modern, and spacious -- even when there are two patients being treated at the same time. Highly recommended.

The in-office experience was wonderful, however the Billing department is absolutely terrible, you have long wait times on hold constantly and they never call back. None of the call center support staff are able to help resolve my issue, sending you to departments that do not take your call, and the management team is not responsive either. They sent a $47 bill to collections without even notifying me of the balance. I will not be returning to their offices.

Dr. Baker and his staff are amazing! They were nothing but kind, respectful and patient. My insurance was giving me a hard time and they handled it for me and it was all taken care of. Dr. Baker and his PA Arik, are extrememly knowledgable and answered all of my questions. The offices are clean and everyone was masked.

There's a very good reason why this clinic has a low rating already. You probably don't want to come here and get yelled at for no reason. Another impression during such short acquaintance - disorganized + not wearing masks while asking you to wear one. They really made me uncomfortable on multiple levels, I had to walk out as I couldn't trust them with my health issue.
