The Community Health Center of Franklin County's (CHCFC) mission is to care for the health of our community, that's why we say "your health, our mission, all welcome". We are here to provide excellent medical care to all of our patients, regardless of insurance status or income. But it's more than that. We recognize that seemingly small barriers can present major obstacles in getting the care that is needed, so we intentionally implement and participate in programs that meet the needs of our community members who are seeking our care.

The Community Health Center of Franklin County has many services that support the care provided by members of our team.
CHCFC has been working collaboratively with this local farm since 2016 to connect patients to healthy food. For two years our two organizations worked together on a research project around diet quality and access to farm shares. Poor diet quality is a leading cause of excess morbidity and mortality-responsible for more deaths than any other risk factor, and over 10% of all disability-adjusted life years in the US.
We are thrilled you have chosen to establish your care with us! Below you will find information about establishing as a new patient with our medical department, our dental department, or both. To get started, please download the 2021 New Patient Registration Packet below. Each form of the packet must be completed in black ink and delivered either by mail or in person to any of our Health Center Locations. After we process your paperwork we will contact you as soon as possible to set up your first appointment.

Had a great experience getting my wisdom teeth done here. Then as Ive needed other dental work its been awful. I had to schedule an appointment a month out for a tooth with a gaping HOLE. Only to now get a call day of the appointment to get it fixed just to hear they cancled my appointment. For a tooth that is just getting worse and now have to wait another whole month!

I've been with CHCFC for the better half of a decade. And they were great! My doc at the time Beth was amazing, personable, and actually listened to your needs. I now have Lisa Miller my first visit with her was rather bleek! Her attitude was awful, she came off as condescending, dismissive and just plane ride. trying to set up a follow up thro CHC's customer service is a nightmare to deal with. and the follow up appointment with a different practitioner went out the window.
It was supposed to be virtual and they were a no show, thank you for wasting my lunch break! They call me 20 minutes late to tell me they are having technical issues. Idk maybe email the client stating that would be solid courtesy. The Doctor who called the. Says she doesn't know what the visit is even for O.o. how do you have an appointment with a patient and now know what the follow up appointments was even for?
How is it not standard practice to at least read up on the last visit! This place has gone down hill in the past 2 years! I used to be able to trust this place and it's practitioners, now I can't!

The best medical center i have EVER been to. All staff was friendly, and efficient. The service myself, and my daughter received was beyond expectations. I would highly recommend this medical center. 5 stars hands down. I'll never forget how kind, and informative everyone i came into contact with was. My daughter's recovery/discharge nurse CADY is a superstar at what she does, and franklin county medical center should be tremendously proud to have such admiral employees. Fantastic job!

I have been seeing Rachel Bensen in the Greenfield office for a few years now and I have never had a bad experience. She is so friendly and truly takes the time to help me in any way she can. I've probably had 13+ appts there and they have always been super nice and on time. There is also a very kind front desk woman who has the best customer service skills ever. Linda may be her name, not sure, (she has long blonde hair), but she is always so incredibly helpful and sweet. The nurses also have always been very kind!
