Services
Your team will include a designated Primary Care Provider that will be responsible for your continuous and comprehensive care. This provider will lead the team of other medical professionals who collectively are responsible for your ongoing care. Your medical team will help coordinate your care ensuring that you receive personalized care using the latest technology and evidence-based medical care.
We are dedicated to providing the best possible care for you, and we want you to completely understand our financial policies. Our office is now accepting CareCredit, the health, wellness, and personal care credit card. Contact our office to pay with this credit card if you are already a cardholder.
To best serve your healthcare needs and to make your visit as pleasant as possible, we have compiled the following information. In an effort to reduce the amount of time you spend in our reception area, all patient visits are by appointment. We do keep appointments for same day scheduling to accommodate urgent and emergency needs of our patients.
These forms are available for download in.pdf format. Forsyth County Schools and surrounding counties require children trying out for school sports to have a physical exam annually. Camps and day cares may also require physical exams every one or two years. Even though we may see your child for other reasons throughout the year, these visits are not considered complete physicals and cannot be used to complete your child's form. If your child desires to participate in school sports or attend camp, and if a physical is needed, please call early to schedule an appointment.

My family and I have been going to Ardmore family practice for our general physician needs for years and it's great! The staff is always extremely kind and friendly, and I've never had any issues with anything in the time I've been going there. I would definitely recommend to anyone looking for a new practice!

My experience have been wonderful here. My daughter and I go here. My daughter has been seen here since she was born. I have seen Dr. Sapp and she is wonderful and compassionate. I regularly see Susan. She is wonderful and compassionate as well. The staff is very caring. When I was experiencing a mental crisis, they were there to help me through it and made sure I had all the support I need. Whenever my daughter needs to be seen unexpectedly, they work out a way to have her seen asap, so she can get well asap. Recommend Ardmore as a PCP. Two thumbs up!

This office is AMAZING. Stephanie is a wonderful doctor who truly cares about her patients. The support staff are always helpful. We've never had to wait more than a day or two to get in and in fact have gotten in the same day multiple times. My entire family are patients here and I am so grateful that we found them.

I have been a patient at this office for 20 years. Dr Amy Sapp sees myself as well as my husband and has seen our 3 children since they were born. She is an excellent doctor. She has a wonderful bedside manner and is great about being frank with children about sensitive issues. She has always worked with me and heard my concerns. I recommend her to anyone!

Been a patient/family member for the best part of 10 yrs and I would not trade Dr. Rosen or Dr. Caroline Grandis for the secret to Bush's baked beans! And I LOVE Bush's, but I love them more and the care they provide is exceptional. You will always leave with a greater understanding of your condition and a plan to ensure the best outcome. The Ardmore Family from check-in to check out is amazing!

Been coming to Ardmore since i was a kid. They delivered 2 of my kids as well. I can honestly say the last year has been a struggle not to up and find another Dr. The front office staff at check in in particular are somewhat rude and careless. Donna makes these faces of annoyance behind staff and patients backs and ive witnessed it several different times.
Today i called and made a 2 o'clock appointment and arrived at 145ish as asked. At 217 i ask why we weren't called yet and the girl being trained i guess says the appointment was actually at 230 which i know i specifically asked for 2 because i had another appointment. I've been told in the past when i called about a sick child that they were too busy to work her in and i had to wait til the following day.
The next day she tested positive for flu and was in bad shape being that she already has issues with her breathing so it could've been worse! You never turn down a sick child running a fever. The wait is extremely long unless you come first thing in the morning or right after lunch. Appointment or not no matter the physician, you will have a long wait. Im extremely disappointed with Ardmore and the way the service has changed in the past few years.

I really like this drs office. I feel comfortable and cared for. I have seen Susan in the past who was wonderful and I am now seeing Caroline. Both made me feel as though I was their only patient and only concern. I suffer from white coat syndrome and yet was made to feel very comfortable with Caroline. I'd like to recommend them as well as say thank you for being there and for your quality care Caroline Grandis. Also, the office staff are all fabulous. Eileen is amazing. She is such a cool and down to earth person and she actually cares about her patients. I highly recommend her. On the other hand, Sue is not someone I recommend. That is if you are a high risk patient with a history of addiction. She treated me like I was nothing more than a junkie even tho I'll have a year clean this March. I had a false positive for a substance and I told her there is no way.
She blamed this false positive on my heart condition and memory problems. Soon to find out on her end that it was in fact a false positive. Since she failed to look at what my heart condition actually is I didn't find out until a month later and by then it was too late to avoid developing it. So like any practice, there will be those doctors who only seem to care about a select few people and treat who they think is worth the treatment. I'm glad I stuck around to become a patient for Eileen. Because she makes up for Sues behavior towards me. Sorry if I spelled Eileen's name wrong, by the way.

I have always always had good experiences in my time with this practice. The front desk workers are matter of fact and business minded so don't expect superfluous smiles and greetings. I have never had to wait long and have always been taken back either early or right on time by the nurses. The nurses have always been warm and jovial when dealing with me and very professional in asking about my visit or taking blood and vitals.
I see Eileen Tague the nurse practitioner under Dr. Sapp, and every visit has been professional. I have never felt rushed during the exam and when I had strep they fully tested for it and then put me on penicillin. You get much better treatment then the express care locations where they just nuke me the strongest antibiotic and rush to the next case.
