He supplies services to patients aged 65 and older in an exceptional practice located at the greater Portland area assisted living and nursing home facilities. Dr. Fazeli makes his visits on site without the need for office appointments.

Medical visits by nurse practitioners (NPs) in the comfort of your apartment at no extra cost. Regular wellness visits every two to three months to stay a step ahead of medical problems and case manage emerging needs. No office appointments necessary; simply request a visit at the facility, or call our office.
Why certain over the counter medications can result in medical issues like falls and delirium. We are working with Durgin Pines in Kittery to change the general approach to fall prevention and interventions by focusing on residents with frequent falls because they need our help the most. There is one issue not addressed enough in most antibiotic stewardship efforts, and that is the availability of dipstick urine testing in nursing homes.
Rural Nursing Homes are very dependent on the few short stay skilled rehab admissions they get from hospitals every month. These few admissions provide a desperately needed Medicare and private insurance payments that can make or break a facility.
