Forest Hills Pediatric Associates is dedicated to partnering with every child/parent-to serve as the child's medical home at every stage of life between birth and adulthood. It's been our privilege to care for children since 1989. There are two ways that parents are introduced to Forest Hills Pediatric Associates: as a first-time parent or as a parent who would like to have their child's care transferred to our practice. We offer a custom approach to both and welcome the opportunity to talk about how we can serve you.

Office INFORMATION Report

Sick children who are already our patients (not first time) will be seen on a first-come, first served basis. Please note, our Walk-In Clinic is for established patients with new-onset illness symptoms. If your child is experiencing a more chronic problem or needs a physical or sports clearance, please call the office to schedule an appointment with one of our providers.

Patient PORTAL Report

The patient portal is a powerful tool allowing you to actively participate in your child's care. Please ensure that a portal account has not yet been set up for your child before you proceed to register without a PIN. If a 2nd parent would like access to their child's chart, please call the office and we will give you a PIN.

Newborns Report

The best time to notify our office is 30-60 days before your due date. Our Patient Ambassador will assist in pre-registration of your child and provide additional helpful information. We recommend you view the Get Acquainted video a month or two before your due date so all are prepared for your child's first visit.

Transfer Patients Report

If you have further questions, please email us at reception@foresthillspediatrics.com. In order to provide the best care possible, we feel it is important to fully understand your child's health history. For this reason, it is important that we first have the opportunity to review your child's health records from their previous pediatrician. We will schedule your first appointment in our office after we have had a chance to review these records.
