A Plus Family Foot & Ankle Center is located in Kent Ohio. The Center is handicap accessible. Use exit 187/13 (Streetsboro). After the toll booth, proceed straight (follow Ravenna sign) onto Route 14 traveling southeast. Turn right at the Route Route 43 intersection and continue south for approximately six miles until you come to the traffic light and the dead end at Haymaker Parkway (State Route 59). Turn left (east) onto Haymaker, go over the bridge, and turn right at Water Street (also State Route 43).
We are a provider for Medicare, Medicaid, most PPO and HMO health plans including, but not limited to, those listed. We are happy to assist you in filing your insurance claims for you. However, your insurance company is responsible only to you, the beneficiary, and requires that you follow up if your claim is not processed in a reasonable amount of time.
