Appointments for physical examinations may be arranged by calling 978-927-4980 extension 7 or when you are in the office for another appointment. If your child has had a recent physical examination and you need a form filled out for school or day care, another examination may not be required. Please provide our office with the form, having filled in all identifying information, and we will mail the completed form to you. Because we have so many requests, there may be a short delay. Please try to plan ahead.
If you download the form from this site, you can either fax (978-922-9115) or mail it to our office. If you have transferred to another practice or have not been seen at our office in over three years, your child's chart is transferred to our storage facility. The complete original chart will be kept in storage for seven years from the last contact date.

Dr. Sleeper is the best! He is knowledgeable and takes the time with you and explains things. He answers all your questions and concerns. He was recommended to me by another person. I'm glad I listened. The nurse line is really helpful. They have same day appointments. Insurance line helps explains the billing and they are super helpful. Only thing I can say is sometimes you cannot get through on the main line so always book for next appointment before you leave!

GCP is phenomenal! From the medical assistants right on up to the Pediatricians! I have been with Dr. Erica Goldstein since my first child was born in 2006 and have since had 3 other children who also see her! The office is open all 365 days and the oncall system for after hours is super helpful! I call the office whenever I have a concern, which is pretty often, and the nurses are ALWAYS super nice and patient! I could go on forever about how much I love this practice and my children's Physician Dr. Goldstein!

My son has been seen by Dr. Suzanne F Graves, M.D. for the past year and a half. Most of our visits were routine and were fine visits. With the exception of them giving him a vaccine by mouth which was told needed to be mixed with a thick liquid due to his aspiration, they refused and gave it to him without mixing it, he choked, stopped breathing, turned blue, and the nurse just froze.
My husband and I jumped up, performed sternum rubs and got him to start breathing again. A few weeks ago my son developed extremely swollen lymph nodes on one side of his neck. Dr. Graves saw him, didn't check any other nodes, didn't run any baseline tests, and put him on Augmentin for 10 days. On the 8th day, the mass doubled. We saw Dr. Graves again who was sure it was an abscessed node and sent us to Beverly Hospital ER to have tests done.
Once there, the Children's Doctor who was on call knew right away by feel that it was not abscessed. After 5 hours at Beverly Hospital, we were directed to Children's Hospital Oncology and Children's Hospital Lymph node clinic where our son is still undergoing testing and treatment. I called GCP to have his records transferred to Children's and the nurse who does that said she didn't see them and that I had to do that myself with a horrible demeanor.
I then called BH records department and they sent everything over right away. We decided to transfer our son out of this practice, but due to insurance, he is not able to go where we wanted. We then requested him stay at Garden City, but to be seen by a different doctor. We received a letter from Doctor Graves today stating "This request to return to our group has been reviewed and deemed inappropriate at this time"!
I would not recommend this practice to anyone, especially if you have a child with any special needs! Dr. Graves was extremely incompetent the last two times she saw our son!

Everyone's so great! Dr. Silva is amazing! I'm a first time mom who calls about every little thing and they're very patient with me. If they cant put me at easy about something over the phone they are always great about getting me in for an appointment. As a first time mom that's so important to me.
