FollowMyHealth is our online patient portal. It allows you to conveniently manage your personal healthcare, securely communicate with your healthcare provider, and more. Note: Because photo identification is required for security purposes, patients cannot sign up online or over the phone. Once you've activated your account, you can access FollowMyHealth by logging in aasthma.followmyhealth.com. If you want to use FollowMyHealth on a mobile phone or tablet, download the FollowMyHealth app from the iTunes Store or the Google Play Store.
To speak to the on-call doctor for urgent medical issues (if you are an existing patient who has been evaluated in this practice within the past 1 year), please call the office numbers or 1-415-750-6756. For regular medication refill, please call during normal business hours so that your record can be reviewed (for your safety) before the refill is authorized.
