Services
Our mission is to provide personalized, comprehensive, family-oriented pediatric care in an independent setting. We help patients navigate the complexity of healthcare decisions, provide coordination of care and follow through, supporting patients beyond the office visit. We focus on physical and emotional health for the overall well-being of our patients and families.
All of our forms are saved in Portable Document Format (PDF). If you do not have a PDF reader already you can download it from Adobe. Permission to Treat- Grants permission for caregivers (other than parents and/or legal guardians) to check-in underage patients. Authorization to Share Information with Family and Friends - Grants access to medical information as specified for patients age 18 and over.
Here are links to most medical documents needed for our local schools. If your child will be starting or changing schools this upcoming year, he or she will need to have a completed entrance form. We will do our best to remind you about the form when you come in for your child's well visit. However, it saves time for all if you have the first page completed when you arrive for your appointment.
Piedmont Pediatrics must receive permission from a child's parent or legal guardian before providing treatment for an injury or illness that is non-life threatening. If the "Permission to Treat" information is not on file with us or presented by the party (baby-sitter, relative, friend) accompanying the child, Piedmont Pediatrics will contact the child's parent or legal guardian before he or she is seen by the physician.

We are so grateful to the entire staff and all providers for caring of our two children since the birth of our first in 2016. Everyone at both locations is kind, knowledgeable, personable, transparent, trustworthy, and most of all caring. I'm extremely sad to have to leave this practice due to moving out of the area. PP will always hold a dear place in our heart, thank you for everything! --Shelley Ellis

My daughter has been a patient at Piedmont Pediatrics since birth (she's now 14). The doctors, nurses, and staff have become like family to us as our daughter has a chronic illness. Dr. Mayo is top-notch, always going above and beyond. Drs. Boersma and Michel have also been exceptional over the years. We can't thank them enough for the kind, caring, and proficient medical care we receive.
As parents we feel valued and part of the medical *team*, our questions and concerns are always welcome. We have had *parent only* appointments where illnesses, treatments, trial medications have been thoroughly explained to us. I would not trust anyone else to handle our child and her illness. My only complaint is that they won't let me become a patient. Apparently 42 isn't considered *pediatric*!

We had always been tremendously happy with this practice until recently. Needed assistance with one child's meds and another child being sick. Received horrible service. After a ridiculous amount of time trying to get answers was told to call back, we asked that they call us since it was their computer causing the issue. They refused to. This being now after 5 pm turned into a disaster.
Never heard back from the treuage nurse. Called back, and after trying another option found out they were actually open. Literally more than 8 calls later, patience was lost and anger set in. Tried to make appointment to bring child in. Receptionist proceeded to inform me my child was not sick, didn't bother to find out it was the other child. Apparently diarrhea doesn't count?
Clearly she became a licensed medical professional between our calls. He was ultimately seen the next day. They told the child, who tried to make his own appointment, that they had none, but when his father called they did. For someone who wasn't sick we couldn't figure out why he was Covid tested. They refuse to see our children anymore because I was unkind to their staff, but the treatment we received was completely expectable to them.
They also could show zero grace to us since my mother in law was just about to be taken off a ventilator and our dog was dying. No I was not my best self, but who would be. Certainly not after refusal to see my child that night because he was not sick according to the idiot they have answering their phones, so he had to continue to be left untreated for another 24 hours.
The other child had to attend school without his ADHD medication. All and all wonderful service if you want to be treated like garbage. Asked for the doctor working to call us, they never did. Emailed the one that we actually really liked to never hear back. So much for doing no harm. They did plenty of harm to my children.

Both my daughters have been seen here for a year. We have been extremely happy with our experience with all the providers. Seriously every one of them have been amazing. They had to help us with an emergency situation with one of our children and went above and beyond for us, even helping after they were technically closed. Highly recommend.

I cannot speak highly enough of everyone here. Dr. Mayo is one of the kindest and most compassionate Doctors you will ever find. Dr. Grice came in on the 4th of July, so that they could get my daughters Bilirubin levels checked. The entire practice is informed and kind and caring and empathetic and I cannot imagine taking either of my daughters any where else.

First of all I want to thank to all of the stuff, nurses and doctors. They served us great. I have been working in hospital for more than 10 years and can tell the doctors in the pediatrics are great. I feel that they are approaching their patient as their child and that is most important things for us. During the time (more than one year) we never felt any discrimination (as foreign) this is another great things for them.
They are treating people as equal. As a local guide I suggest Piedmont Pediatrics. We already moved to another city. We already missed them. Again Thank to Piedmont Pediatrics.
